Consent to Share Your Video Testimonial

Thank you for sharing your experience with MDT Leadership.

Before anything is posted, I want to be clear about how I’d like to use it and give you full control over what happens.

I’m requesting permission for MDT Leadership to use your video testimonial, including the full video, edited clips, a written transcript, and still images from the recording. It may appear on: mdtlead.com, social media (LinkedIn, Instagram, and others), email newsletters, proposals, and/or presentations.

Unless you say otherwise, you give me permission to use your full name, title, and organization alongside your testimonial. If you would prefer something different, email me at madeline@mdtlead.com and I’d be happy to adjust.

A few things to know:

• I may edit for length and clarity. I won’t change the meaning of what you said.

• Your testimonial covers only what you say in this video. I never share anything else from our coaching work together.

• This is voluntary, unpaid, and has no effect on our coaching relationship.

• You’re responsible for confirming that sharing is okay with your employer, if that applies to you.

• The testimonial will stay in use until you ask me to remove it. If you change your mind, email madeline@mdtlead.com and I’ll take it down from my website and social channels within 2 business days. I can’t retrieve copies others have already shared or saved.
